Care shouldn't stop at the hospital door.

Ask Health exists so that the moment a patient leaves the hospital is the moment their care gets stronger — not weaker.

Our mission

Managing patients in place, so facilities can deliver their best.

Ask Health supports hospitals and post-acute care facilities in managing patients while in-place so they can deliver the best possible care. Our unique approach is proven to drive successful outcomes and reduce readmission rates.

We do this by leveraging technology to help your clinical staff manage patients regardless of the setting — a skilled nursing facility, a rehabilitation center, or the patient's own living room.

The hybrid model

In-person care, multiplied by technology.

Neither telehealth alone nor traditional visiting specialists can keep up with today's patient acuity. Our model combines both — deliberately.

Bedside first

Our specialists examine patients in person, build relationships with facility staff, and author the care plan with full clinical context.

Digitally continuous

Between visits, technology keeps specialist eyes on every patient — monitoring changes and supporting your clinical staff in real time.

Escalation that knows the patient

When something changes, the in-person visit is triggered by a clinician who already knows the patient — not a stranger reading a chart.

How it works

From admission to thriving at home.

01

Partner & assess

We embed with your facility or hospital, understand your patient population, and identify where specialty gaps are driving transfers and readmissions.

02

One integrated care plan

Our multidisciplinary team — neurology, psychiatry, palliative care, pain management and more — evaluates patients and aligns on a single plan with your staff.

03

Manage in place

Patients are treated where they are. In-person rounds plus continuous digital monitoring mean small changes get caught early — before they become ambulances.

04

Measure & improve

Reduced readmissions, stronger quality metrics, and confident staff. We report on outcomes and keep raising the bar together.

Our team

The people behind the care.

Ask Health is led by clinicians and operators who have built specialty care programs across hospitals, facilities, and homes.

What we believe

The values behind the model.

Patients over process

Every protocol we build starts from the same question: what does this patient need to recover — and what's in the way?

Collaboration, not competition

We make facility staff and attending physicians stronger. Our specialists extend your team — they never work around it.

Outcomes you can measure

Readmission rates, quality metrics, family satisfaction. If it matters to your patients and your business, we track it and own it.

Let's talk about your patients.

Whether you run a facility, lead discharge planning, or care for a loved one — we'd love to show you what managing patients with confidence looks like.
